Payment Policy

We aim to keep care affordable while ensuring the highest quality service. Payments are due at the time of service, with alternative arrangements considered case-by-case. A detailed cost estimate for procedures or surgeries will be provided during your consultation.

Insurance Information

Please complete the insurance section on the new patient intake form. Insurance reimburses patients, not the office. You are responsible for deductibles, co-insurance, or uncovered balances, plus any collection costs, attorney fees, or court costs for unpaid accounts.

Laboratory and Surgical Services

Treatments requiring dental lab services need a minimum down payment at the initial visit. Patient portions for services, including surgery, anesthesia, and lab fees, are due at the time of service.

Account Balances

Accounts unpaid after 90 days will be sent to collections. A service charge applies 30 days after your first statement, based on your balance. A $30 fee is charged for returned checks.

Appointment Policy

We value your time and strive to maintain an efficient schedule for all patients and providers.

Late Arrivals

Arriving over 20 minutes late to your appointment may require rescheduling to keep other patients on time.

Consultation Cancellations/No-Shows

We understand that emergencies or obligations may require you to miss an appointment. However, failing to cancel a non-surgical appointment at least 24 hours in advance may result in a $50 fee, which is not covered by insurance.

Surgery Cancellations/No-Shows

Surgical cancellations made less than 48 hours in advance or failure to follow pre-operative guidelines may incur a fee of $500 or the full estimated surgery cost, whichever is less. This fee is not covered by insurance. Rescheduling may require paying the estimated patient portion upfront.

Emergency Considerations

We understand that emergencies, sudden illness, or unforeseen circumstances occur. Patients are encouraged to contact our office as soon as possible in such cases. Fees may be waived at the discretion of management upon review.

Provider-Initiated Cancellations

If a surgeon must cancel due to an emergency, no fees will apply. We will notify you promptly and reschedule at your convenience.
