For your convenience, we accept cash, check, Visa, Mastercard, Discover and American Express. We offer outside financing through Care Credit as well. We deliver the finest care at the most reasonable cost to our patients. Therefore, payment is due at the time services are rendered unless other arrangements have been made in advance. Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
We make every effort to keep down the cost of your care. Payment is due at the time of service. Arrangements may be considered depending upon special circumstances. An estimate of the charges for any procedure or surgery you may require is given at your consultation. If you have any dental and/or medical insurance, please complete the insurance portion on first page of form.
Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some companies pay fixed allowances for certain procedures and other pay a percentage of the charge. It is your responsibility to pay a deductible amount, co-insurance or any other balance not paid for by your insurance company. You will be responsible for all collection costs, attorney fees, and court costs.
For treatment that requires dental laboratory services, a minimum down payment will be required at the initial appointment. All patient portion for surgery is due at the time of service.
It is not our office’s policy to have accounts due past 90 days. Therefore 30 days after your first statement there will be a service charge added depending on your account balance. All accounts past 90 days will be sent to collections. There will be a $30.00 fee for all returned checks.
Scheduled Appointments for Consultations
We understand that delays can happen however we must try to keep the other patients and doctors on time.
If a patient is more than 20 minutes past their scheduled time, we will have to reschedule the appointment.
Cancellation/No Show Policy for Consultations
We understand there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient getting treatment.
If a consultation appointment is not cancelled at least 24 hours in advance you will be charged a $50 fee; this will not be covered by your insurance company.
Cancellation/ No Show Policy for Surgeries
Due to the large block of time needed for surgery, last minute cancellations can cause problems and add expenses to the office.
If a surgery appointment is not cancelled at least 48 hours in advance, or not following pre-op guidelines given to you before surgery you will be charges $500 or the entire cost of your surgery, whichever is less; this will not be covered by your insurance company. You may have to pay your estimated portion, of your procedure cost, to reschedule another surgery appointment.
